How to View Violations in Licensed Assisted Living Homes in Michigan
Learn how to find, read, and understand Michigan assisted living violation reports, including what established violations mean and why some facilities show “Not Applicable".
When a complaint about an assisted living home is filed with the state of Michigan, a licensing consultant investigates the allegation. If a rule violation is substantiated, the findings are documented in a Special Investigation Report and published online by the state.

How to Find Violation Reports
Our website lets you view inspection and violation records for every licensed assisted living facility in Michigan, if available. The buttons shown below take you directly to the government's website where reports are published.
The first thing to look for is if the facility has had a rule violation in the last year (shown below). This appears when you expand the Show all facility information tab. Click the learn more button to view the published reports.
You can also view all available reports even if they did not have a violation established in the past year by clicking on the Check State Inspection & Investigation Reports button shown below.
Both of the buttons shown above take you to the official government page for the facility which contains the Inspection and Special Investigation Reports (if they exist).

How to Read Violation Reports
The most important words to look for are VIOLATION ESTABLISHED or REPEAT VIOLATION ESTABLISHED and read the supporting details. Some reports have more than one violation established so look through the entire document. Below is an example of a violation established
Why do some facilities show Not Applicable for violations?
Some Michigan facilities are allowed to operate under a license exemption rather than an AFC or HFA license. These facilities do not operate under the same AFC/HFA licensing rules, so they may not have the same LARA inspection, violation, or special investigation reports. An exempt facility showing no violations does not always mean it has never had an incident or problem. They are regulated differently or exempt from AFC/HFA licensing.
